  • Verifiable Driving History
  • DOT Physical Card
  • Minimum Auto Liability/General Liability of $1,000,000 (Listing Aggregate Transport Corp as an additional insured)
  • Worker’s Comp Insurance Certificate (if more than one commercial vehicle is listed on your certificate and someone else operates the vehicle)
  • Authorization to Release Drug and Alcohol Tests
